For Expanding The Data System For The ACCESS Sonoma County Initiative

Overview
The Sonoma County Department of Health Services will lead the implementation of the ACCESS Sonoma County Initiative across all its Safety Net departments to improve efficiency and services. The need for this Initiative became urgent after the October 2017 wildfires, which resulted in increased homelessness and exacerbated existing problems, including mental health challenges, chronic poverty, substance use dependency, and significant health problems. The initiative provides an integrated data hub, for use by newly formed interdepartmental teams, to coordinate services for clients who have multiple needs.
